Please fill out the form below completely and accurately. Please be
advised that the LEGAL limit of passengers is 4. Please have pets in a cage when riding in the taxi cab. If you do not
have your pet in the appropriate pet cage you will not be picked up. Rates are 2.50 meter drop and 2.50 per mile after
that. Waiting time is 25.00 per hour. Please make sure after you exit the vehicle you have all of your belongings.
I am not responsible for lost or stolen items left in the taxi. All found items will be taken immediately to the
Rainbow/Executive office. Thank you for your patronage.
